Are you a Registered Nurse looking for an opportunity to make a lasting impact outside of a traditional bedside setting? Join our team as an RN Case Manager and help individuals with intellectual and developmental disabilities live safely and independently in their homes and communities.

In this role, you’ll partner with clients, families, caregivers, physicians, and interdisciplinary teams to provide clinical oversight, education, and care coordination while building meaningful, long-term relationships.

What You’ll Do

  • Provide clinical case management and nursing oversight for individuals receiving waiver services.

  • Conduct assessments through home visits and phone consultations to evaluate health needs and support quality care.

  • Develop and maintain individualized care plans, nursing protocols, medication administration records, and other required clinical documentation.

  • Perform nursing delegation in accordance with Colorado regulations and educate caregivers and Direct Service Providers on delegated tasks, medications, and clinical best practices.

  • Collaborate with physicians, families, service coordinators, and community partners to coordinate comprehensive, person-centered care.

  • Monitor client health status, identify changes in condition, and communicate recommendations or obtain updated physician orders as needed.

  • Support new client onboarding by assessing clinical needs, medical equipment, medications, and delegated nursing tasks.

  • Ensure documentation is accurate, timely, and compliant with agency policies, HIPAA requirements, and state and federal regulations.

  • Serve as a trusted clinical resource for clients, families, and care providers by answering questions and providing guidance during business hours.

  • Participate in team meetings, ongoing education, and quality improvement initiatives.

What You’ll Bring


Required Qualifications

  • Current Registered Nurse (RN) license in the State of Colorado.

  • Bachelor of Science in Nursing (BSN).

  • Current CPR certification.

  • At least one year of nursing experience in an acute care setting.

  • Strong clinical assessment, communication, and organizational skills.

  • Valid Colorado driver’s license, reliable transportation, and proof of insurance.

Preferred Qualifications

  • Experience in nurse case management or home health nursing.

  • Experience supporting individuals with intellectual and/or developmental disabilities.

  • Supervisory experience in a healthcare setting.

  • Familiarity with Colorado waiver services and applicable state and federal regulations.
