POSITION
I/DD Waiver Coordinator


POSITION PURPOSE

To support PASCO families, clients, and caregivers enrolled in I/DD medicaid waivers (DD, CES, SLS) while adhering to state and PASCO policy.


KEY DUTIES AND RESPONSIBILITIES

  • Coordinate and oversee Individualized Service Plans (ISPs)
  • Collaborate with healthcare providers and social workers to ensure comprehensive care
  • Advocate for clients and ensure their needs are met in a person-centered approach
  • Stay informed on I/DD waiver regulations and updates to ensure compliance
  • Empower clients and families throughout the waiver process by providing guidance and support
  • Communicates regularly with PASCO service leaders to ensure consistent messaging on the advantages that PASCO provides to clients and caregivers
  • Updates Client Relationship Management (CRM) system regularly to build upon networking opportunities and appropriate follow-up
  • Receive, track, and audit incoming documentation from our clients and caregivers
  • Create and maintain client and provider schedules in electronic health records system

Preferred skills

Physical or Other Requirements:

  • Medium detailed work:
    • Access to a reliable transportation, either public transportation or an automobile with a valid Colorado driver’s license and insurance.
    • Requires walking or standing a significant amount of the time.
    • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
    • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing.
  • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).


JOB TYPE Full-time

LOCATION Hybrid: 2 day onsite (Wed/Th) in our Lakewood offices with other days working remote and visiting our families and clients.  There is occasional flexing to cover evening and weekend appointments as needed.

COMPENSATION $20 - $23/hour + mileage reimbursement
