Job Type: Full-time
Location/Schedule: Hybrid — 2 days onsite (Wednesdays & Thursdays) at our Lakewood, CO office; remaining days remote and in the community. Occasional evening/weekend flexibility required.
Compensation: $22–$25/hour + mileage reimbursement

PASCO is seeking a full-time Bilingual Service Coordinator to join our mission-driven team. In this vital role, you’ll work closely with Spanish-speaking individuals and their families to help them access life-changing services and support. What You’ll Do:
Support and guide Spanish- and English-speaking individuals and families receiving I/DD services
Coordinate care services and attend regular home visits, service plan meetings, and team conferences
Educate families about Medicaid waiver programs (CES, SLS, DD) and available resources
Partner with nurses to ensure health and safety needs are addressed in service plans
Work with Direct Support Providers (DSPs): help schedule training, conduct quality checks, and provide ongoing support
Build community partnerships and represent PASCO at outreach events
What You Bring:
✔ Bilingual (Spanish-English) – required
✔ Experience in case management, family services, or client care coordination
✔ Passion for supporting individuals with disabilities and their caregivers
✔ Familiarity with Medicaid waiver services (preferred)
✔ Strong interpersonal and organizational skills
✔ Commitment to cultural responsiveness and advocacy
