Job description

Job Type: Temporary with potential for long-term employement based on performance and organizational needs.
Location/Schedule: Hybrid — 2 days onsite (Wednesdays & Thursdays) at our Lakewood, CO office; remaining days remote and in the community. Occasional evening/weekend flexibility required.
Compensation: $22–$25/hour + mileage reimbursement

What You’ll Do:

  • Coordinate care services and attend regular home visits, service plan meetings, and team conferences

  • Educate families about Medicaid waiver programs (CES, SLS, DD) and available resources

  • Partner with nurses to ensure health and safety needs are addressed in service plans

  • Work with Direct Support Providers (DSPs): help schedule training, conduct quality checks, and provide ongoing support

  • Build community partnerships and represent PASCO at outreach events

What You Bring:

  • ✔ Experience in case management, family services, or client care coordination

  • ✔ Passion for supporting individuals with disabilities and their caregivers

  • ✔ Familiarity with Medicaid waiver services (preferred)

  • ✔ Strong interpersonal and organizational skills

  • ✔ Commitment to cultural responsiveness and advocacy
