POSITION

RN, Start of Care

POSITION PURPOSE

The Start of Care Nurse assists individuals with long term care needs, which will maintain them in the least-restrictive and supportive environment of their choosing. Manages support services and monitors clients’ care. Acts as an information and resource guide for clients, family members, and other professionals. Assists in Single Entry Point program evaluation, outreach, and resource development. Performs within regulations, guidelines, and deadlines established by State agencies.


Key Duties and Responsibilities

  • Coordinates with health care team members to assess, plan, implement, or evaluate client care plans
  • Work with the PASCO intake team to onboard our new clients
  • Work with physicians offices and healthcare portals to obtain start of care medical documentation
  • Collaborate with area Case Management Agencies to ensure proper service delivery
  • Partner with a team of nurses to support clinical questions and as-needed questions for clients, families and coordinators
  • Ensure compliance of regulatory documentation before an individual starts care
  • 35Other duties as assigned by PASCO’s nursing administration

Job requirements

  • Experience:
    • Experience working with individuals with disabilities or chronically ill population preferred
    • Experience providing case management services, interviewing, and counseling preferred
  • Education and/or Licenses:
    • BA or BS in Nursing preferred
    • BLS/CPR required
    • Unencumbered license to practice in the state of Colorado as an RN

Physical or Other Requirements (with little or reasonable accommodation):

  • Sitting some of the time in front of a computer screen or paperwork, in combination with walking, standing or exerting force.
  • May require walking or standing more than 1/3 of the time.
  • May require exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
  • Requires talking, hearing and detailed work with fingers, such as repetitive typing, and may require stooping, kneeling, crouching, grasping, lifting, reaching, pushing
  • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).

Pay Range: $34 - $43 DOE
