Reports To: Admissions Nurse Manager

Pay Range: $82,000 – $92,000 annually (based on experience)

What You’ll Do:

  • Complete Start of Care assessments and evaluations per PASCO, State, and Federal guidelines

  • Consult and coordinate with physicians and interdisciplinary teams

  • Educate clients and families on home health strategies and resources

  • Document timely and accurately using electronic health records

  • Recommend devices or services that improve quality of life

What We’re Looking For:

  • Active RN license in Colorado (required)

  • BLS certification (required)

  • Experience:

    • Working with individuals with disabilities or chronically ill population preferred

    • Experience providing case management services, interviewing, and counseling preferred

  • Ability to adapt to change and work independently

  • Reliable transportation and willingness to travel (average 30%)

  • Bonus points for: BSN degree, Spanish fluency, experience with trach care, G/J tubes, ports, or pediatric populations

Physical or Other Requirements (with little or reasonable accommodation):

  • Sitting some of the time in front of a computer screen or paperwork, in combination with walking, standing or exerting force.

  • May require walking or standing more than 1/3 of the time.
