At PASCO, we are growing fast and looking for a compassionate and experienced Service Coordinator to join our team! In this role, you will play a key part in guiding individuals with I/DD and their families through essential services, ensuring they receive the care and support they need to thrive.

What You’ll Do:
Support clients and families receiving I/DD services to navigate their care and achieve their goals
Coordinate services, complete supervisory home visits, and attend service planning meetings
Provide guidance on Medicaid waivers (CES, SLS, and DD) and help families understand available resources
Collaborate with nurses to ensure health and safety needs are met in service planning
Work closely with Direct Support Providers (DSPs), scheduling training, coordinating meetings, and ensuring quality care
Attend community events and build relationships with agencies to strengthen support networks
What We’re Looking For:
✔ Experience in case management, client coordination, or customer service
✔ Background in intellectual and/or developmental disability services (preferred)
✔ Knowledge of Medicaid waivers and caregiving is a plus!
✔ Bilingual (Spanish/English) is preferred, but not required
✔ A problem-solver and strong communicator who is passionate about improving lives
