Pay, Bonus & Benefits

  • $36–$42/hour base pay

  • Monthly performance bonus: Percent-to-goal, paid for meeting PASCO Nurse Coordinator metrics, including for IHSS roles

  • Mileage/toll reimbursement

As an IHSS RN Coordinator, you'll play a vital role in designing and overseeing personalized care plans, ensuring every client receives the high-quality, compassionate support they need at home or in the community. You’ll collaborate with experienced Case Managers, champion best practices, and empower caregivers through hands-on clinical guidance and training.​ This role covers clients in the north east region of Colorado and is a hybrid role requiring the chosen candidate to work onsite out of our Lakewood offices x2 days per week on Tuesdays and Thursdays.

Key Responsibilities

  • Develop and implement agency plans of care tailored to individual client needs, safety, and preferences.​

  • Train and validate attendant competency in essential health maintenance activities.​

  • Review incident reports to identify trends, create solutions, and improve safety.​

  • Collaborate across departments to ensure care is seamless, compliant, and client-focused.​

  • Serve as a trusted resource for caregivers and clients, offering support and expert guidance.​

  • Colorado-licensed RNs with at least 1 year of experience and a passion for empowering others.​

  • Pros with prior supervisory experience (preferred) and the flexibility to adapt and grow alongside their clients and team.​

Qualifications

  • Current and unencumbered Colorado Registered Nurse license (required)

  • Current CPR certification (required); First Aid certification is not necessary but highly preferred​

  • Strong documentation and collaboration skills; comfort with Microsoft Outlook and EMR systems.​

  • Bachelor of Science in Nursing highly preferred (not required)

  • Experience with disability care a HUGE plus.

  • Spanish speaking (highly preferred)
