RN Coordinator

PASCO is seeking a dedicated In-Home Support Services (IHSS) & Personal Care Services Nurse Coordinator to join our team!  As an Nurse Coordinator, you will play a crucial role in coordinating and overseeing healthcare services for our clients located in Lakewood, Littleton, and Denver metropolitan area. If you are a compassionate and organized individual with a passion for healthcare, we would love to have you on our team! Join us in making a difference in the lives of our clients and their families.  In this role, the chosen candidate will:

POSITION:

Nurse Coordinator

POSITION PURPOSE:

Provide direction, coordination, supervision and implementation of professional and supportive services to Home Care Agency (HCA) consumers. Monitor services provided to IHSS consumers, provide training to attendants and ensure consumer satisfaction and safety. Complete and submit all required documentation.

Key Duties and Responsibilities include:

• Communication with team and clients

• Medical documentation

• Annual In-Person Supervisory Visits 

• Perform record audits

• Education to clients, employees and families

• Utilizing Carevoyant software program

Experience:

• 1 year experience practicing as an RN (required) with additional experience in acute care 

• Med/surg experience highly preferred

• Working knowledge of disabled community is preferred. A person with a "disability" is someone who has a physical, mental or communication disability that substantially limits one or more of the major life activities such as caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning and working.

• 1 year of supervision experience overseeing caregivers and other care providers

• Spanish speaking (highly preferred)

Education and/or Licenses:

• Current, unencumbered state certification to practice as an RN in the state of Colorado

• BSN highly preferred

• Current CPR certification is required; First Aid certification is not necessary but preferred

Pay Range: $36 - $42/hour DOE + $1,000 sign-on bonus  

Location: Primary caseloads in Littleton and Centennial.

Work Environment: Hybrid; Remote and in the field with Tu/Th in office
