Job Type: Full-time (32+ hours/week)

Pay: $36-$42/hour DOE + $5,000 sign-on bonus (full-time only) + mileage reimbursement
Location: Denver Metro

We are seeking a compassionate and skilled Registered Nurse Case Manager who is fluent in speaking, reading, and writing in Spanish to support Spanish-speaking clients in the Denver Metro area. In this home health role, you will oversee the comprehensive coordination of care and deliver skilled nursing visits to a dedicated caseload. This is an ideal opportunity for a nurse looking to gradually grow from part-time to full-time within a flexible, supportive environment.

KEY RESPONSIBILITIES

  • Conduct home visits every 60 days (or as needed) to reassess care needs for CNA and RN services

  • Deliver skilled nursing care within RN scope of practice

  • Complete orders and resumption of care assessments within 48 hours of client hospital discharge

  • Educate clients and caregivers, providing guidance in Spanish on diagnosis, symptoms, and home care

  • Ensure accurate documentation and clear communication in both English and Spanish

  • Coordinate with interdisciplinary teams and ensure alignment of care plans and physician orders

  • Provide required CNA competencies and training

  • Maintain professional communication with providers, clients, and families

REQUIREMENTS

  • Must be fluent in speaking, reading, and writing in Spanish

  • Active and unencumbered Colorado RN license with 1 year nursing experience required

  • Current CPR/BLS card

  • Valid driver’s license and reliable transportation

  • Previous home health experience highly preferred

  • Excellent communication and time-management skills

  • Experience working in the disabled community highly preferred
