POSITION

Nurse Manager, Waiver Services


POSITION PURPOSE

Lead and manage Waiver Service Registered Nurse Coordinator team. Ensure clinical care delivery is high quality and safe for the client. Collaborate with Quality Assurance Department and Regulatory Compliance Committee to ensure clinical care delivery adheres to PASCO policy, State and Federal Regulations. Facilitate communication between Waiver Services Nurses and VP of Clinical Services. Align PASCO’s strategic goals with Waiver Services RN Coordinator team goals.


Reports to: VP of Clinical Services


Key Duties and Responsibilities

  • Direct and supervise Waiver Services Registered Nurse Coordinator Team.
  • Motivate Waiver Service RN Coordinators to strive for professional excellence through coaching and ensuring standards of care.
  • Perform annual performance evaluations and day to day job coaching and discipline.
  • Oversee clinical operations to ensure clinical care is delivered in accordance with PASCO Policy, State and Federal Regulation.
  • Collaborate with the administrative team and supplemental nursing staff in order to assure the health and safety needs of individuals receiving waiver services.
  • Effectively work with all departments in the office as well as with community partners and case management agencies.
  • Research and maintain a resource library for best practice clinical care protocols to support the needs of individuals who receive services.
  • Attend community-based events and meetings in order to provide outreach and collaboration with members of the community as well as county and state representatives and departments.
  • Provide Medication Administration instruction (QMAP) to newly hired direct service providers.
  • Provide instruction to Waiver Nurse Coordinators on nurse delegation to direct service providers.
  • In collaboration with Clinical Educator, develop a training program and training tools to support onboarding Waiver Service RN Coordinators.
  • Address compliance concerns and create Performance Improvement Plans to improve program improvement initiatives.
  • Collaborate with VP of Clinical Services and Director of Compliance on key indicators of program health and take initiative in creating and implementing the program’s Performance Improvement Plans.
  • Develop forms and tools, in collaboration with PASCO’s Regulatory Compliance Committee for Waiver Service Registered Nurse Coordinators to implement in day to day job duties.
  • Notify nurses of hospitalizations.
  • Notify Medical Records and/or designee of upcoming discharges.
  • Other duties as assigned in support of quality service to individuals receiving services.

Key Performance Indicators:

  • Quality Assurance Team survey findings and outcomes.
  • Timeliness and accuracy of Waiver Services Nurse Coordinator paperwork
  • Consumer feedback on nursing staff
  • Consumer retention
  • Engagement, morale and retention of employees, especially community care nurses

Job requirements

Experience

  • 1-2 years of Home Health nursing (or equivalent) required
  • Experience managing nurses within a Home Healthcare company preferred

Education and/or Licenses

  • Current unencumbered license to practice as a Registered Nurse in the state of Colorado
  • Bachelor’s degree in nursing preferred
  • Master’s degree in related field preferred

Knowledge, Skills & Strengths

  • Thorough to expert clinical and logistical knowledge of home care service delivery
  • Thorough to expert knowledge of Medicare, State & Licensure Home Care Agency and any other governing body rules and regulations for care delivery and documentation
  • Knowledge of state Medicaid waiver programs
  • Leadership and management skills, e.g. ability to monitor progress, audit performance and provide effective feedback, influence, motivate, manage conflict and change
  • Strong to expert clinical skills, ability to provide or act as a resource in cases of substantial or specialized nursing skills
  • Appreciation of how all areas of the business work together and affect each other; specifically, the short- and long-term ramifications of clinical decisions on PASCO’s operations, finances, advocacy and regulatory relationships
  • Ability to communicate effectively and proactively, ensuring that all members in the organization have accurate information in a timely manner
  • Ability to identify, assess and prioritize legal, financial and clinical risks in their domain, as well as efficiently minimize, monitor and control those risks
  • Strong organization and time management skills, ability to maintain one’s own schedule and meet deadlines
  • Knowledgeable and capable with Microsoft Outlook, Word, Excel, PowerPoint, and experience with Electronic Health Records systems
  • Ability to work with a diverse population

Physical or Other Requirements:

    • Medium detailed work:
      • Requires walking or standing a significant amount of the time.
      • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
      • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing.
      • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).
    • Access to a reliable transportation, either public transportation or an automobile with a valid Colorado driver’s license and insurance.

LOCATION/SCHEDULE: This position is hybrid working both in-office (Lakewood, CO) and remote.

PAY: 75K - 90K DOE
