What You'll Do
Provide direct, hands-on skilled nursing care in client homes: medication administration, wound care, monitoring vital signs, and patient assessments.
Serve as a care coordinator, collaborating with families, providers, and multidisciplinary teams to ensure optimal outcomes.
Assess and adjust care plans in response to client needs and changing conditions.
Travel throughout Denver metro and Aurora to deliver exceptional in-home care.
Who You Are
Experienced LPN with at least 1 year in nursing (home health preferred).
Holding a current, unencumbered Colorado LPN license.
Graduate of a state-approved School of Practical Nursing.
Valid driver’s license, insured vehicle, and readiness to commute.
Current CPR certification (BLS preferred).
Experience with the disability community is a valued asset!
Core strengths: stamina, attention to detail, organization, empathy, excellent communication, adaptability, and teamwork.
Skilled in documentation, problem solving, and maintaining strict patient confidentiality.
