Key Responsibilities:

  • Provide high-quality, compassionate care to patients in their homes.
  • Develop and implement individualized care plans.
  • Monitor patient health and update medical records.
  • Collaborate with healthcare professionals and family members to ensure optimal patient outcomes.
  • Administer medications and treatments as prescribed.

Job requirements

Experience:

  • 1-3+ years of experience practicing as a Private Duty Nurse (PDN) providing in-home care (preferred).
  • Documented experience and skill set matching client/family needs.
  • Home Health nursing experience (highly preferred).
  • Experience with trach care, G-Tube, and ventilator care (highly preferred).

Licensure/Education:

  • Must hold a current and valid RN or LPN license in the state of Colorado (confirmed through DORA).
  • Graduate of an accredited school of nursing.
  • Current CPR certification is required.
  • Active and current driver’s license/insurance.

Knowledge/Skills/Strengths:

  • Clinical Judgment: Ability to independently collect and interpret data about a client, arrive at a conclusion, and identify the appropriate action. This involves problem-solving, decision-making, and critical thinking rooted in clinical knowledge and experience.
  • Client Service: Ability to establish and maintain relationships with clients, providing professional, appropriate, compassionate, and empathetic care.
  • Maintaining Commitments: Ability to consistently fulfill commitments, show up when promised, and maintain strong communication.
  • Collaboration: Ability to maintain a positive attitude, demonstrate flexibility, and balance self-interest with the interests of the agency.
  • Knowledge: Understanding of State, Federal, and Licensing rules and regulations for care delivery and documentation by Home Care Agencies. Awareness of unique issues related to care delivery for people with a variety of disabilities.
  • Communication: Ability to communicate effectively both orally and in writing.
  • Technical Skills: Ability to operate a computer, familiarity with Microsoft products (specifically Outlook), and ability to receive and process work in an Electronic Health Record (EHR) system.
  • Problem-Solving: Strong problem-solving skills and the ability to adapt to changing situations.

Physical Requirements:

  • Requires walking or standing a significant amount of the time during the shift.
  • Capability to assist clients with mobility or transfers.
  • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
  • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing, twisting at the waist.
  • Requires close visual acuity to perform activities such as preparing and analyzing data and figures, viewing a computer terminal, extensive reading, and visual inspection involving small defects, small parts, and/or operation of machines (including inspection).

Other Requirements:

  • Must be willing to provide in-home nursing care to patients of varying ages and medical complexities.
  • Reliable and able to follow shift schedules.
  • Ability to accurately document patient care and maintain medical records.

JOB TYPE part or full-time

SHIFT SCHEDULE Day shifts. Part of full-time availability working typical 10 or 12 hour shifts.

LOCATION determined by client's location

PAY RANGE 
$23 - $38/hour DOE and licensure.
