POSITION:
HCA Waiver Coordinator

POSITION PURPOSE:

To support PASCO families, clients, and caregivers enrolled in IHSS and Personal Care Services while adhering to state and PASCO policy.

KEY DUTIES AND RESPONSIBLITIES:

  • Coordinate with case management entities throughout the state to help support individualized care plans utilizing established assessments, client needs, and observations.
  • Completes on-site personal care supervisory visits according to the time frames specified in the governing body rules and regulations and agency policy
  • Participates in creating and reviewing Care Plans, and makes any necessary updates
  • Observes Personal Care Workers (PCWs) and Homemakers performing tasks and adhering to the care plan
  • Provides performance management feedback on PCWs and Homemakers to Personal Care Supervisors (PCS)
  • Understands differences between skilled and unskilled tasks and limitations of Personal Care Workers and Homemakers per governing body rules and regulations, and assures that PCWs and Homemakers are practicing within those limitations
  • Collaborates with other departments including Medical Records, Prior Authorization Team, HR, and Intake
  • Assists in creating, distributing, and maintaining all program/s documents
  • Establish and maintain ongoing community relationships with individuals, organizations, state entities and related institutions to build business relationships
  • Reviews and assists with compliance regarding program/s obligations
  • Communicates regularly with PASCO service leaders to ensure consistent messaging on the advantages that PASCO provides to clients and caregivers
  • Updates CRM (Client Relationship Management) system regularly to build upon networking opportunities and appropriate follow-up
  • Support, establish and maintain relationships with PASCO families, clients and caregivers through continuing phone calls, telehealth meetings, and home visits
  • Receive, track, and audit incoming documentation from our clients and caregivers
  • Create and maintain client and provider schedules in electronic health records system

  • Feedback from individuals receiving services
  • Maintaining compliance with due dates, adhering to PASCO and state policy

Job requirements

QUALIFICATIONS:

Knowledge, Skills or Abilities

  • Experience with case management or client care coordination
  • Experience with Medicaid or Home Healthcare experience a plus
  • Minimum of 2 years proven success in an account management building successful partnerships
  • Excellent verbal and written communications skills
  • Proven skills of customer satisfaction
  • Ability to successfully operate in a fast paces dynamic environment experiencing growth and expansion
  • Excellent organization and time management skills, ability to maintain ones own schedule and meet deadlines
  • Strong computer skills including Microsoft Office Suite, Outlook
  • Experience with Electronic Health Records systems preferred
  • Self-motivated and self-directed
  • A passion to serve our clients
  • Must be willing to travel to clients homes in the community for check in visits required
  • Bilingual (Spanish and English) highly preferred

Physical or Other Requirements (with little or reasonable accommodation)

  • Sedentary detailed work
  • Sitting most of the time in front of a computer screen, on the road, or paperwork
  • Walking and standing are required (up to 1/3 of the time)

  • Requires talking, hearing and detailed work with fingers, such as typing

PAY RANGE: $20 - $23/hour

Location/Schedule: Hybrid work schedule.  Tu/Th in office (located in Lakewood, Colorado or Colorado Springs) all other days remote.
