Location: Denver or Colorado Springs (Hybrid)
Schedule: Full-time | Mon/Tue in office, Wed–Fri remote
Compensation: $21–$24/hour + bonus potential

Join PASCO (Personal Assistance Services of Colorado) as a Care Navigator—a role where your voice, empathy, and drive directly improve lives.
As a Care Navigator, you’ll guide individuals and families through the Medicaid long-term care process, connecting them with services and empowering them to live more independently. This position is ideal for someone who is energetic, goal-oriented, and skilled at building meaningful connections—especially over the phone.
What You’ll Do
Serve as the first point of contact for individuals and families exploring services with PASCO.
Educate people with long-term disabilities on Colorado Medicaid Long Term Services and Supports (LTSS) and how PASCO can help.
Conduct initial outreach via phone, email, online inquiries, in-person events, and home visits.
Build strong rapport, ask the right questions, and listen deeply to understand each family’s unique needs.
Document interactions using Salesforce and other tools; maintain and manage your own schedule.
Coordinate internal team members to assist with onboarding or assessments, and keep prospects engaged through the process.
Reinforce PASCO’s reputation as a leading provider of home-based services with heart and integrity.
What You Bring
Excellent verbal and written communication skills
Energetic, well-spoken, and persuasive—especially over the phone
Bilingual in Spanish is a big plus!
Strong listening skills and ability to ask targeted, helpful questions
Self-directed with solid time management and multitasking abilities
Detail-oriented, efficient, and able to work quickly with accuracy
Strong customer service mindset and relationship-building skills
Previous experience in one or more of the following is helpful:
CRM systems (like Salesforce)
Healthcare documentation or EMRs
Home Health, Medicaid, or LTSS services
Proficiency in Microsoft Outlook and Teams
Comfortable working autonomously while collaborating with a supportive team
Thrives in a fast-paced, growing environment
Physical & Other Requirements
Sedentary, detail-focused work—frequent typing and screen time
Must be able to exert up to 10 lbs occasionally
Frequent talking, listening, and data entry
Visual focus required for data review and documentation
