POSITION
Care Navigation Manager
POSITION PURPOSE
To lead the Care Navigation (intake) team to ensure a smooth admissions process for clients and families, focusing on responsiveness, relationship-building, and service excellence.
Key Duties and Responsibilities:
- Provide leadership and support to the Care Navigation team.
- Oversee the start-of-care process, ensuring timely onboarding of clients.
- Maintain communication with community partners and case managers.
- Respond promptly to inquiries and address client concerns.
- Guide clients towards appropriate services or resources.
- Collaborate with other departments to optimize service delivery.
- Act as a subject matter expert on regulations and procedures.
- Document interactions in internal systems.
- Qualify leads and schedule client meetings.
- Continually enhance client and partner experiences.

Job requirements
- 60Excellent communication skills.
- Ability to connect with people and ask targeted questions.
- Strong time management and multitasking abilities.
- Persuasive, goal-oriented, and self-motivated.
- Proficiency in Microsoft Office Suite and Outlook.
- Experience with CRM/Salesforce and healthcare documentation systems preferred.
- Knowledge of Medicaid or Home Healthcare is a plus.

- Proficiency in Microsoft Office products and electronic health record systems.
Physical or Other Requirements:
- Sedentary detailed work
- Sitting most of the time in front of a computer screen, on the road, or paperwork.
- Walking and standing are required (up to 1/3 of the time).
- Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
- Requires talking, hearing and detailed work with fingers, such as typing.
- Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).
- Examples: Positions that require sitting at a desk talking on the telephone, or entering data or processing paperwork that require only occasional walking to meet with others or retrieve items of negligible weight
PAY $55K - $65K annually DOE + quarterly bonuses
