Job Type: | Full-Time
Location/Schedule: Hybrid – 2 days onsite (Wednesdays & Thursdays) at our Lakewood, CO office; remaining days remote and in the community. Occasional evening/weekend flexibility required.
Compensation: $22–$25/hour + mileage reimbursement

What You’ll Do:

  • Support and guide individuals and families receiving I/DD services through Medicaid waiver programs

  • Conduct regular home visits, participate in service plan meetings, and maintain contact with interdisciplinary teams

  • Educate families on services available through the CES, SLS, and DD waivers

  • Collaborate with nurses to ensure health and safety needs are reflected in care plans

  • Provide oversight and guidance to Direct Support Providers (DSPs), including scheduling training and conducting quality checks

  • Represent PASCO at outreach events and build connections in the community

What You Bring:

✔ Experience in case management, social work, or client support services
✔ Familiarity with Medicaid waivers or long-term care systems (preferred)
✔ Strong organizational and communication skills
✔ Ability to work independently and collaboratively in a hybrid environment
✔ Commitment to equity, inclusion, and person-centered care
